PERSONAL DATA PROCESSING CLARIFICATION TEXT
As Renew & Revive and its personnel, we would like to inform and enlighten you about your personal data provided to us within the scope of the services we offer, in accordance with Article 10 of the Personal Data Protection Law No. 6698 ("KVKK"). We would like to assure you that we take maximum care to ensure the privacy and security of all personal data you provide to us, and that all necessary technical and administrative security measures are taken to protect this data.
DATA CONTROLLER AND REPRESENTATIVE
As Renew & Revive, acting as the data controller, we may process, record, store, classify, update your personal data within the scope of the purposes explained below, and disclose/transfer it to third parties in cases permitted by law and/or limited to the purposes for which it is processed, in compliance with legal principles and good faith.
YOUR PROCESSED PERSONAL DATA
Your personal data includes:
- Identity Information: Your name, surname, T.C. identification number, place and date of birth, marital status, gender, and other identity data that can identify you.
- Contact Information: Your address, phone number, email address, and other communication data obtained through various communication methods.
- Financial Data: Bank account number, IBAN number, credit card information, billing details, and other financial data.
- Health Data: Personal health data you share and declare for beauty and cosmetic purposes (data about diagnosis and operation history, information about medications, body mass index, health issues mentioned in consent forms related to the services and operations).
- Web browsing information: IP address, browser data, surveys, form information.
- Visual and Audio Data: Pre- and post-operation photographs, and recorded images during the application.
- Other Information: Profession, wedding anniversary, birthday, etc.
PURPOSE OF PROCESSING PERSONAL DATA
Your personal data is processed based on your explicit consent and/or other legal grounds as stipulated in Article 5/2 of KVKK. Considering the services provided by Renew & Revive, your personal data is processed for the following purposes:
- Fulfillment of service-related transactions and ensuring that the relevant persons benefit from the services provided.
- Planning and carrying out customer-specific sales and marketing activities (if there is consent for commercial electronic communication, providing personalized promotions, opportunities, and benefits based on preferences and needs).
- Execution of production and operation processes for goods/services.
- Execution of sales processes for goods/services.
- After-sales support for goods/services.
- Execution of customer relationship management processes.
- Activities for customer satisfaction.
- Execution of advertising/campaign/promotion processes.
- Marketing processes for products/services.
- Providing information to authorized individuals, institutions, and organizations.
- Billing for the services you receive.
- Providing information about the services you receive, complementary services, and new services.
- Notifying you of your appointment if you schedule one.
- Providing information sharing, reporting, and fulfilling notification obligations as required by relevant legislation in case requested by public authorities and institutions.
- Fulfilling the obligations of storing information and documents arising from legal regulations.
Your personal data is processed within the conditions of personal data processing as mentioned in Articles 5 and 6 of Law No. 6698.
METHOD OF COLLECTING PERSONAL DATA AND LEGAL GROUNDS
Your personal data is collected by Renew & Revive through verbal, written, or electronic channels (such as documents, contracts, and forms in physical environments; through the corporate website, emails, internal software, audio recordings, and call center systems in electronic environments). Your data will be processed based on your explicit consent or, where applicable, the following legal grounds:
- When required by the provisions of laws and regulations in force in the Republic of Turkey (e.g., the Turkish Code of Obligations, Commercial Code, Labor Law, Occupational Health and Safety Regulations, Tax Procedure Law, Consumer Law).
- When necessary for the execution of a contract to which you are a party.
- When necessary to fulfill legal obligations.
- When personal data has been made public by the data subject.
- When necessary for the establishment, exercise, or protection of legal claims.
- When processing is required to pursue our legitimate interests, provided it does not harm your fundamental rights and freedoms.
Special categories of personal data are processed based on your explicit consent or, in cases where consent is not required under the law, for the following purposes:
- Non-health-related special categories of personal data may be processed without explicit consent in cases specified by law.
- Health-related personal data may be processed without explicit consent to protect public health, provide preventive healthcare, conduct medical diagnosis, treatment, and care services, and plan and manage the financing of healthcare services you receive.
TRANSFER OF PROCESSED PERSONAL DATA
Your personal data will be processed in compliance with the law and good faith principles, with accuracy and, when necessary, up-to-date, for legitimate, specific, clear, and lawful purposes. The data will be stored for as long as necessary for the purposes of processing, in line with the principles stipulated by the applicable legislation.
Your personal data may be transferred to domestic and/or foreign business partners, suppliers, insurance companies, notaries, banks, financial institutions, legal and tax advisors, relevant public authorities, and private persons for the continuation of Renew & Revive's activities. These transfers will comply with Articles 8 and 9 of the Law No. 6698 and will be made in line with the purposes mentioned above.
Your personal data will be protected as professional confidentiality within the framework of Law No. 6698 and relevant regulations, and will not be shared with third parties/institutions/organizations except in cases of legal obligations.
RIGHTS OF THE DATA SUBJECTS
As the owner of personal data, you have the following rights under Article 11 of the Law:
- To inquire whether your personal data is being processed,
- To request information about the processing if your data has been processed,
- To learn the purpose of data processing and whether your data is used in accordance with this purpose,
- To learn about third parties, domestic or foreign, to whom your data has been transferred,
- To request the correction of incomplete or incorrect personal data,
- To request the deletion or destruction of personal data,
- To request notification of the actions taken to third parties to whom the data has been transferred,
- To object to the result of automated processing if it negatively affects you,
- To request compensation if you suffer damages due to the unlawful processing of personal data.
CONTACT INFORMATION FOR REQUESTS
In case of written requests, the “KVKK Application Form” available on our website, signed in wet ink, can be submitted in person with a document verifying your identity or with a notarized power of attorney to DEĞİŞTİRİLECEK address. The form can also be sent by a notary.
For electronic requests, you can send your application to [email protected] using a secure electronic signature, mobile signature, or via the email address you previously provided and registered in our systems, in accordance with Article 5 of the “Communiqué on the Procedures and Principles of Application to the Data Controller.”
Your requests will be responded to in writing or electronically as soon as possible and no later than thirty days after the request is received, depending on the nature of your request.
