EXPLICIT CONSENT TEXT ON THE PROCESSING OF PERSONAL DATA

Effective date: 17 October 2026

I have read the Privacy Notice prepared by the data controller FKT GÜZELLİK AKADEMİ KOZMETİK DANIŞMANLIK LTD. ŞTİ. ("Renew & Revive"). Each of the consents below is separate and optional; giving one does not require giving the others. Not giving consent does not prevent me from getting information or receiving services. I may withdraw any consent at any time by writing to [email protected] or by using the opt-out option in a message sent to me; withdrawal applies to processing from that date on. In case of any discrepancy, the Turkish version prevails.

1. 1. Sharing my contact details for ad measurement and transfer abroad

I give my explicit consent to the irreversibly hashed (SHA-256) form of my phone number and email address that I share in the website forms, the chat or messaging channels, information about the advertisement through which I reached the website (ad click identifiers, campaign information, the page visited, IP address and browser information) and the outcome of my request (information request, appointment, purchase and its amount) being shared with Meta Platforms Ireland Ltd. / Meta Platforms, Inc. and Google Ireland Ltd. / Google LLC to measure the effectiveness of advertising and improve ad delivery, and to the related transfer abroad.

This consent is given with the "For ad measurement" checkbox in the website forms. Consent for analytics and marketing cookies on the website is collected separately in the cookie preferences screen.

2. 2. Processing of my health data

I give my explicit consent to the health data I declare (allergies, medication, skin condition, any health condition preventing a treatment, etc.) being processed and stored by the relevant specialists and authorised staff of Renew & Revive so that beauty and care treatments can be performed safely. This data is never used for advertising or marketing and is not shared with third parties.

This consent is collected separately at the salon before the treatment.

3. 3. Before and after photos

I give my explicit consent to before and after photos being taken and stored to follow up the treatment. Publishing my photos on the website or social media, even with my face unrecognisable, requires my separate written approval.

4. 4. Preference analysis for personalised offers

I give my explicit consent to the services I have received and am interested in being analysed to prepare offers and campaigns for me. Sending these offers to me by SMS, email or WhatsApp additionally requires my commercial electronic message approval.
